Cowboys for Codi, Inc.

Agreement to Participate;
Assumption of Risk and Release

   WHEREAS, the undersigned (the "Participant") wishes to participate in a benefit rodeo and/or a benefit barrel race (the "Event") to be organized and conducted by Cowboys for Codi, Inc.; and in consideration of Cowboys for Codi, Inc.'s action in allowing the Participant to participate in such Event:

   The undersigned acknowledges that during the Event that the Participant will participate in certain rodeo activities and that certain risks and dangers exist. These include, but are not limited to, the hazards of being near and/or handling rodeo livestock, handling tame livestock and operating rodeo arena equipment. The undersigned further recognizes that these risks may also include loss or damage to personal property, injury or fatality due to accidents which may occur during the Event. Participant further understands that in participating in the Event, Participant will be exposed to elements of nature, including temperature extremes and possible inclement weather.

   In consideration of the right to participate in the Event, Participant has and does hereby assume all of the above risks and any other ordinary risk incidental to the nature of the Event, including risks which are not specifically set forth above and will hold Cowboys for Codi, Inc., its Shareholders, Directors, Officers, Employees, Agents and/or Associates, and their heirs, executors and administrators, harmless from any and all liability, actions, causes of action, debts, claims, and demands of every kind and nature whatsoever, whether for bodily injury, property damage or loss or otherwise, which may arise now or in the future from or in connection with the Event or participation in any other activities arranged by Cowboys for Codi, Inc., its Shareholders, Directors, Officers, Employees, Agents and/or Associates. The terms hereof, and Participant's signature on this document shall serve as a release and assumption of risk, and shall bind Participant's heirs, representative, executors, administrators, successors and assigns.

   I, the Parent/Legal Guardian, of the above signed Participant understand the nature and risks of the Event being sponsored by Cowboys for Codi, Inc. and, as the Participant's Parent/Legal Guardian, grant permission for his/her participation in the Event.
